Fill a 350x19 canvas with two colours using the gradient tool
Add in a picture to the left side
Add text with the font "visitor TT2 BRK"
Stroke the font - 1px
Get the Elipitical Marquee tool and make a semi cirle on the top or bottom of the bar.
Fill it with white and lower the opacity to give it a 3d look
Add scanlines by Edit >> Fill and select it in patterns
If you dont have any open a 3x3 canvas and zooom in all the way and get the pencil toool Set on 1px and draw a diagonal line across it. Then EDIT > DEFINE PATTERN and name it scanlines.
New layer
Get the rectangular Marquee tool and select the whole thing
Edit>>Stroke >> inside 1px and any colour you want.
If i explained it right it should look like this
In this one i used different scanlines but you get the picture.
