Did you fully format your hard-drive? If not do so (dont do quick format) because if you have one of the virus's that hide in a disks boot sectors and you dont do a full format it could still be there. Also it could be your ethernet cable going from your computer to the internet source that is causing the lag spikes. Other than that I cannot think of anything.

It is either:
A.) Virus that you didnt get rid of
B.) Your hard-drive is going bad.
C.) Your modem or what ever you use to connect to the internet is going bad.
D.) Your cable's for said connection are going bad.
E.) Your graphics card is going bad.
F.) Your CPU is going bad.
G.) Your windows installation could be slightly corrupted.

If it is not one of these then I have no clue. I am going to school to be a network technician (and I am a certified hardware technician).

But really as far as "lag" goes I dont think a corrupt windows install would only cause the game to lag. I just put that possibility in there just incase. I more than likely that it is a hardware issue.

Here do this for me:

Go to star menue, find RUN, in run type cmd. click ok
Then in that box type ping http://www.yahoo.com, copy that and past it in your comment so I can take a look at your responce times. That might help me figure out what is wrong. Also if you can find it get the ip for SRO server and ping that.
