how do you get + things on ur items?

like if i have a +1 Helm, how do i make it +2?
please explain >.<

You need an item called an elixer. There are 4 different types of elixers for each different type of gear you have(Shields, Weapons, Protector, Accessory). You also need a lucky powder that matches the degree of item you have. Well you don't NEED it but it is recommended. Press "Y" to open the alchemy screen, put the item in, add the elixer and powder and click okay. Sometimes it fails and reverts back to 0. Unless you have an astral and it is above +4 it will do this.
