if your capped then gold is easy to find as long as you don't stop playing.
more npc item drops on high level that's why they have that big amount of gold.

I've got 31M on my account, of which 20M I made myself. It's really easy imo ... play Capture the flag, get 30 tickets and sell them for around 750K Do it 3 times a day and you have 2.1M. But I'm a wizard, many kills for me :p

I have no idea, HelloKitty was an ex-unionmember, the screenshot was posted at our union site. I am a zero at PS (and at the rest photo editing software as well), although that amount of gold can be possible by virtue of the guy's past.
