Quick Question About An Application Form

This is my first Job Application :D And I'm kinda stuck on one part of the form. It might sound stupid, but I want to ask anyway to make sure.

Where it says "Full name and address of employer" under "Previous Employment", is that the address of the Company, or the address of the employers actual house?

Also, I need to write in my National Insurance number. I know I have one, but I lost the card where it written down on. Anyone know where I can find it again? EDIT: I mean where can I get my number from, not where can I find the card :D

A fast answer would be awesome, thanks.

the address of your previous employer means the physical location of the workplace you would go to(where your work is and its address) , not your bosses personal address =D, if this is your first job then you can leave that section blank ..

as far as the Insurance # , cant help you there , never had to write that down on any of my applications..

what you could write is "Will Provide Upon Employment" as so you wont have to give out SUCH personal information when you don't know whether you will receive the job or not..

OK thanks for clearing that up. The problem I think I would have if I write "Will Provide Upon Employment" is that they might just see it and think, if he hasn't bothered to give his insurance number, we wont bother employing him - there are so few vacancies (if any), and so many applicants so they can afford to drop applicants for the tinyiest thing.

EDIT: Also it says in that section, "Notice period required". What does that mean?
