KSRO Residence Registration Number and Name?

KSRO Residence Registration Number and Name?

I downloaded KSRO and trying to register it asks me these two things. Ive been searching for these two things for a while, but havent found anything. Could someone please tell me these numbers and the name.Thx

go to the SRO Korean website and register there ok
