Since you want the psd, you should know somethings about photoshop.. so why dont do it yourself ? There is a tutorial for that sig

Out of curiosity, why would you need the PSD ?
...sorry I can't think of a liable reason to why some one who is making a request would need something like that unless he is planning to do something unethical with it.

Just have them make it textless, then you can add whatever name you want.
