Sig/Avatar please

Using that image ( i think its from guildwars but the model is similar to a warlock in WoW (in the new tier)) I would like a sig,MED - 375x125, using that picture and the words "God of Death"

the avatar can be just the head or whatnot, I don't care much as long as it matches.
