When alchemy fail?

I know that when item fails in an enchant, one of the options u'll get is:
Item restarts from the zero like its(+0).
But now to the other option:
Durability fall to 0, or in another words, item is destroyed(gone).
Now the question is:
It will be gone only if it was +5 or +4?
I need a 100% sure answer.
no offence but, dont tell me maybe this and maybe that, i need u to be sure 100% of your answer please.

when ur upping a weapon u get a chanse it gets destroyed when you are going for +5 and higher, so trying to get a +7 without immortal is something you should never do.

going till +5 is no problem chanse u get ur weapon destroyed there is nihil (thats what i've read anyways)

after +5 there is a chance of it getting destroyed and very little chance of it being +6 or +7 i know this from experience destroyed lvl 32+5 weapon. Made a whole new lvl 32+7 now its godly has far as i care lol and will fetch me a high price when i want to sell it
i dont believe +int and such there is a chance for the weapon to be destroyed

Only the other day i was trying to +5 my 72 Shield and it broke going to +5 :-(

Its the first time ive had an item break on me going to +5
had a few break trying to+6
