berserk? how does it refill?
what determines how the berserk bar fills? like is it based on the # of enemies killed or damage or what?

It is based on luck.
Sometimes you can get your all the balls back consecutively.
Sometimes it will take you hours of constant killing to fill it up.
There does not seem to be any type of pattern to it.
In general, fast killing of many monsters will help you get berserk faster.
But in the end it is random.

yeah, if you watch the yellow/blue orbs of exp flow around you, if you see a fat red orb go to you, one circley thing on your zerk gauge gets refilled.
So killing mobs gets you zerk-orbs. The number of mobs needed to kill to attain a zerk-orb is based on luck, as stallowned said.
