I'm not sure about the chances. Never been released. Though I'm sure the higher "+" it is, the chance decreases dramatically.
If it fails, then it will surely go back to 0.
If you fail enchanting if the item is already +4 and over, there is a small chance the item may break(disappear completely) or have the durability reduced. But immortal and steady prevents that.

you can only test the chances of a weapon by trying it. if its hard to + at first then it will be less lucky.
i have experienced the following senarios.
1. i made a weapon +3 with 3 elixirs, i tryed to make it +4 and it failed back to 0. I made the same weapon again +3 with 3 elixirs and tryed to make it +4, and it failed again. at this point you know its time to give up. or waste some more money to make it +3 so you can atleast get some money back when you sell it.
2. another example. i made a weapon +6 with 6 elixirs, i went to make it +7 and the weapon got destroyed, leaving me with no weapon at all.
after a while you will learn your own pattern when to stop or when to go for more.
what i do now, is simple. if i want to make a weapon +3. i just get 3 elixirs and do it 1 after another, its the best way for me to see if the item is lucky or not so lucky.
