so i plan to make it to +5. but i dun wanna take the risk.
so ill get 1 astral and 1 immortal on it.
wad will happen if i upgrade from +4 to +5. if i fail. wad will happen?

If you have Astral It won't go down to +0 but get back to +4 (in your case stay at). If you have Immortal your Item won't reduce its durability.

Also by activating the astral to save it from going below +4 an astral charge will be used. Put astral back on before trying to go to +5 again.

Not quite true, Immortal will save it from being destroyed completely. Steady is the attribute needed to prevent a reduced durability.

can an astral work alone without any immortality on the equipment?

No. There has to be at least the same amount of immortal attributes in order to put astral on it. So let's say, immortal is 1, astral can be 1 as well. immortal is 2, astral can be 2. Immortal is 0, no astral possible.
