What is the condition for a Party Mob to appear?

What is the condition for a Party Mob to appear?

I know we have to be in a party for a party mob to appear.
But I still have a few questions.
1) Do we have to be in a 8 man pt? or a 4 man pt?
2) When we are in a Pt, do all Pt members need to be in close proximity? or not?

I was hunting in a 4/4 man Pt earlier, and every1 was in different areas and i see no PT mobs at all.
