Hmm, it doesn't say lvl 4 anymore. Only with the deletion scroll. But I'm still pretty sure it is lvl 4.

Its somewhat easy. Find the icon you want (needs to be 16x16 PIXELS) and either resize it, or save it as "guildmark". Make sure that you save it as a windows 24 bitmap. DO NOT NAME IT GUILDMARK.BMP. IT will show up as guildmark.bmp.bmp. If you need help resizing, Post it here and ill do what i can.
