I never had that trippy stuff happen but I did buy a lvl 5 challenge stone once and it NEVER got in my inventory. Figured it was like the chat lag and would show up eventually.....2 months, no stone. WTF is my stone :P , when im like lvl 80 it will just pop up in there and I will be like "woot" :D
Back after 3 years....

it's quite obvious that it's an arrow with the wrong icon displayed. Your Data.pk2 file may be corrupted. Or maybe it's an ingame glitch that looks for the wrong filename on accident.
