McCain is a conservative republican. Another word for conservative is "right wing" (left wing for liberals). Last week or so, his "conservatism" was challenged by his party because some thought he was actually liberal (republicans love conservatives). The cartoon depicts his plane (himself) losing his right wing (his conservative ideals), yet he gains altitude (still leads the GOP nomination) despite losing his 'right wing'.
