Does anyone know how to write and CV or do you know where i can find a template?

Personal information (name, date of birth, etc.)
Education and qualifications (schools you went to, courses you took, with dates)
PC-skills (important for my work; 2d&3d software)
Work experience (all places i worked at so far, with dates)
Interests (hobbies)
Additional information (languages you speak, drivers license, personal abilities, etc.)
Prestations (like winning a design contest, etc.)

do you write a reason for leaving employers?

Depends what country, in England most companies ask for 2 references, either personal or old employee. If you've just come out of college, could be a tutor/teacher. If they ask for references you have to give them, and if there is any problems, like you have lied about your references and they find out they could fire you.

Actually it's only your Resume, but in most cases you make a cover letter as well.

No, a CV is a curriculum vitae. There's a big difference between a CV and a resume. CV's focus on your academic experience: degrees, certificates, publications, conference attendances, conference presentations, teaching or lecturing experience, research, etc. A resume is focused on your work experience: past jobs, duties, responsibilities, training, specialized skills, qualifications, etc. Education is included on a resume, but in a normally shorter section just as a highlight.
There are only a few reasons why you would want to submit a CV and not a resume. You are a recent graduate and you have very little real world experience. You are applying for an academic or research position.
From your questions you seem wary that a potential employer will call one of your past employers and find out you were fired, dismissed, or something generally bad. There are ways to write your resume around problem employers, and ways to address these types of situations with a prospective employer. Perhaps if you explain what the real issue is the advice you recieve will make more sense.
This is valid for the U.S., if you live somewhere else you'll have to find this out on your own. In many states, such as Florida, a past employer can only provide information concerning your dates of employment and whether or not you are eligible to be rehired. In many cases they are not allowed, by law, to discuss why your employment was terminated, or their personal feelings about the matter. Your application to a potential employer serves as a release form that allows them to verify the employment records you provided, so if you do not include an employer they cannot contact them. Obviously, big gaping holes in your resume will create questions, you should be prepared to answer them.
